Professionals do not always have the opportunity to familiarize themselves with the client’s health history. During home visits, the focus is on the tasks and procedures specified in the care plan. The need for care may vary as the client’s functional ability and health status change, in the short or long term, without the nurse noticing all the changes and the related notes.
